These Terms of Service govern your use of Sugam Polyclinic's website, WhatsApp service, diet chart registration form, online consultation booking, and patient dashboard. By using our services — including submitting the diet chart registration form or booking an online video consultation — you agree to these terms.
Sugam Polyclinic provides medical consultation, appointment booking, online video consultation, and personalized diet planning services, accessible via WhatsApp and our online systems.
Your diet chart is prepared based entirely on the information you provide. We do not conduct an in-person physical examination as part of this specific service. The accuracy, safety, and suitability of your diet chart depend directly on the accuracy and completeness of the name, age, height, weight, health conditions, medications, allergies, and other details you submit through our form.
Your diet chart is prepared and reviewed by our doctor based on the information you provide, and is intended as general dietary guidance for the goals and conditions you have described. It is not a substitute for an in-person medical diagnosis, treatment plan, or ongoing clinical supervision.
If you have a medical condition, are taking medication, are pregnant or breastfeeding, or have any concern about whether a dietary change is safe for you, you must inform us accurately in the form and should consult the doctor directly before making significant changes to your diet.
Online video consultations are held daily within a fixed time window, as shown at the time of booking, and are subject to change or unavailability due to the doctor's schedule.
The consultation fee is non-refundable once payment is completed, including where the patient is unavailable, unreachable, or does not answer at the scheduled time. In such cases, Sugam Polyclinic will reschedule the consultation to another available date at its own discretion — the fee itself will not be refunded or reversed for a missed consultation.
It is the patient's responsibility to be reachable on WhatsApp, on the phone number used for booking, at the scheduled consultation time. If the doctor is unable to reach the patient after a reasonable attempt, the consultation will be treated as missed for that day, and rescheduled by our team as described above.
If Sugam Polyclinic is unable to conduct the consultation for reasons on our end (e.g. doctor unavailability), we will reschedule the consultation at no additional cost.
Information and diet plans provided through our WhatsApp service, registration form, or dashboard are for general guidance and do not replace professional in-person medical diagnosis or treatment. Always consult the doctor directly for medical concerns requiring urgent or specialized attention.
Appointment bookings made via WhatsApp are confirmed automatically and assigned a unique reference number. We reserve the right to reschedule or cancel appointments when necessary, and will notify you accordingly.
A registration fee of ₹299 (5-day plan) or ₹699 (14-day plan) applies to the diet chart service, and a consultation fee of ₹299 applies to the online video consultation service, payable at the time of submitting the respective registration or booking form. Diet plans are generated, and consultations are scheduled, only after successful payment verification, except where waived at the doctor's discretion.
Because your diet chart is prepared and delivered digitally shortly after submission, the diet chart registration fee is non-refundable once payment is completed and your information has been submitted, except where required by applicable law. The online consultation fee is likewise non-refundable once payment is completed, as described above under Online Video Consultation.
You agree to provide accurate, complete, and truthful information when interacting with our WhatsApp service, registration form, booking form, or dashboard. You understand and accept that any inaccurate, incomplete, or misleading information you provide may directly affect the safety, accuracy, and appropriateness of the diet chart, consultation, or other recommendations we provide, and that we are not responsible for outcomes arising from information you did not disclose or disclosed incorrectly.
We may update these Terms of Service from time to time. Continued use of our services after changes constitutes acceptance of the updated terms.
